She knows him as Masen in New York, that's why she didn't make the connection of him being Cullen. The times she used it was when she wanted to spite him, and that time in bed where she accidentally called him that. So there, hope I clarified some things for you.;)

Bella's POV
At every corner and crevice, a torch was mounted onto the stony walls. The flames seemed to flicker menacingly, mocking us. Massive court guards stood at each torch with weapons strapped across their chests and more at their sides. Their cold, dark red eyes always ended up snapping to me, watching every little step I took as we passed by them. I couldn’t help but wonder if the three judges would look just as cold and unforgiving as the guards. My heart stuttered in my chest at the mere thought of losing Edward. I quickly pushed the ill thought away as we continued our trek further into the cold, dimly lit cave. With each step, the air grew colder and denser. I shivered next to Edward. He quickly took off his grey pea coat and draped it over my shoulders.
I glanced up at him and smiled my gratitude. Even with the thick sweater I wore, I had been freezing ever since we entered the ominous cave. The entire atmosphere down here felt chilling and foreboding yet the beauty of the stalagmites helped put me at ease slightly. The ceiling of the cave had to be at least 200 feet high. We came to a stop by a stone archway guarded by two heavily armed court guards dressed in gray uniforms. Their dark eyes narrowed on me before flashing to Edward.
The burlier of the two guards spoke. “Edward Anthony Cullen?” he asked in a gruff tone.
“Yes.”
“You’ll need to get changed and restrained. Follow me.” The burly guard turned on his booted heel and stomped away, expecting us to follow.
Edward pulled me behind him and snaked his arm around my waist when the tunnel grew more narrow and darker. Not even the flickering flames helped aid my sight. The torches were placed too far apart. I leaned against Edward, letting him guide me. The walk through the tunnel didn’t last long. We came to a stop in front of a small cavernous room filled with shelves full of uniforms and black chains. In the center of the room, a huge platform made of stone stood ominously as the centerpiece.
The court guard stalked over to a shelf and grabbed a pair of baggy cloth pants that were burgundy in color with black stripes. “You will need to put these on while I prepare your restraints and the bite cuff. After you finished changing into the convict garb, sit down on the restraint table over there and wait for me,” he instructed in an icy tone while pointing to the big stone platform. “You can change behind that wall over there, the changing divider. Just leave your clothing on the shelf when you’re finished. If you’re found innocent, I will escort you back here to change back into your clothes. If you’re found guilty, you will remain in this uniform I have given you to be immediately escorted to the maximum security prison by the prison guards assigned to you. Any questions?”
